Referral Charter

1. I will not accept, or solicit, referred patients for ongoing dental care patient.
2. I will endeavor to provide a high standard of care to you and I value your referral.
3. Your patient will be given a clear written treatment plan and fee estimate at the initial consultation.
4. If during the course of treatment an alternative or additional treatment is required, I will notify you before continuing.
5. My preference is to return the patient to you with the root canal permanently sealed.
6. After completion of treatment and also after the review I will promptly forward copies of the final radiographs to you for your records.
7. Initial endodontic consultation appointment IS FREE, any radiographs cost £7.
