Middle mesial canal

This a recent case referred because of discomfort from the 46.
The original RCT was completed in Germany approximately 20 years ago.
Access revealed a Resorcinol-formaldehyde resin "Russian Red" obturated distal and mesio lingual canal.
After removal of this material with ultrasonics, the mesio buccal canal was filed and shaped. Further instrumentation revealed a middle-mesial canal which curved distally and explains the radiolucency in the furcation region.
Literature states that as many as 15% of lower molars have middle-mesial canals.
Canals prepared with a Red Reciproc file and obturated with 150 GP/AH+ using Otura, i.e. "Squirted".
