A lower first premolar was identified as having a PARL on radiographic examination for a replacement crown. Patient is 67 years with MH NAD.

Although the root anatomy looks straightforwards lower premolars can have very complex root anatomy, which needs to be identified under magnification, or by CBCT pre op.Dam was placed and Race 123 was used with hypochlorite, EDTA and CHX irrigation in sequence, ensuring no mixing of irrigants. One canal was identified and prepared to size 35/04. Obtuation was with 35/04 taper gp system b and obtura 150, AH+ as sealer.
